I noticed you made a comment about the textbooks. I was teaching at a famous juku (cram school) in Japan. The school used textbooks written partially in Japanese. Specifically, the books used katakana as the basis for English pronunciation. If you know anything about the Japanese language, you know that katakana is used for writing and pronouncing words borrowed from other languages, but mostly from English. The use of these textbooks perpetuated the use of incorrect pronunciation. More about this in an upcoming post.
